Instructions for online visa application form

(1) Before starting your application, please make sure you have the Internet Explorer version 6.0, 7.0, 8.0, or Mozilla Firefox version 3.6 or higher, or Safari 5.0

(2) In order to preview, print or save your online form, please make sure that you have installed a version of Acrobat Reader with Simplified Chinese Kit.

(3) Please do not use IE's buttons but the "NEXT" or "PREVIOUS" button at the bottom of the page if you want to move forward or backward between pages.

(4) You can save the form in this website at any step by clicking "SAVE" button at the bottom of the page. However, this website can only save your form no more than 30 days.

(5) Please make sure all items are filled out. Incomplete forms will make you unable to go to the next step.

(6) You can click the question mark behind each item for help information which will be found at the right side of the page.

(7) Please print and sign the completed form, and submit it to the Chinese Visa Application Service Center.

(8) Please make sure all information is complete, updated and accurate. The form cannot be modified once you click the "CONFIRM & PRINT" button.

(9) Please note that the online application form does not mean that your visa will be issued by the Chinese Consulate-General. The final decision on your visa which is made by the Consulate-General might be different from your application.

Terms&Conditions

Chinese Visa Application Service Center (the Center) is responsible for accepting visa applications for visiting mainland China from ordinary passport holders who are Canadian citizens and citizens of other countries possessing Canadian permanent residence or work permits (hereinafter referred to as 'you'), collecting visa fees on behalf of the Chinese Consulate General and returning passports. Applicants holding diplomatic and service (official) passports or eligible for applying for China's diplomatic, courtesy and service visas, or those applying for a visa /entry permit to the Hong Kong Special Administrative Region of China or Macau Special Administrative Region of China should still apply directly to the Chinese Consulate General (the Consulate General). The Center welcomes you using its services.

By submitting an application, you will be taken to have read, understand and accepted all of the Center's terms and conditions (the Terms):

(a) under which the Center provides visa application services (the Services) to you;

(b) which include but are not limited to this notification and those that are published in the Center's official website (www.visaforchina.org), policies, fee schedules, flyers, brochures and other official materials; and

(c) which impose, among other things, obligations on you to make payments to the Center and to indemnify the Center and limit your rights and the Center's liability in some cases.

Approval of Visa by the Consulate General

The Center has no involvement in the visa assessment and decision-making process. Whether a visa is to be issued or not to be issued, and what type of visa to be issued as well as its period of validity, number of entry and duration of stay shall be decided by the Consulate General in the light of China's relevant laws and regulations. An applicant will be required to pay a service fee to the Center regardless of approval or disapproval of his or her application by the Consulate General. The service fee is non-refundable.

The Consulate General has the right to request the applicant to provide additional supporting documents or to come to the Consulate General for an interview.

Information provided by you

To facilitate assessment by the Consulate General of your application, you are kindly required to:

(a) prepare all the documents that are required and ensure they are genuine and credible;

(b) fully fill out the visa application form with true and accurate information (application form for an under-age child should be signed by one of his or her parents or guardian and the relationship indicated).

Please carefully check all the printed information on your visa when you collect your passport. If you have any queries about the visa, you should report it to the counter staff immediately. You will be responsible for any consequences arising from the following:

(a) Your visa becoming invalid and therefore not being able to be used as a result of your own fault or any act by a third party acting on your behalf;

(b) In the event that you are refused to board an aircraft or to enter China due to your failure to check all the information printed on your visa including your name’s spelling, birthday, nationality, your visa's number of entry, period of validity and duration of stay as well as the number and validity of your passport before departure for China;

(c) Your visa cannot be used due to your own other reasons.

Submission Time

Please submit your visa application at an appropriate time so that your visa would not expire or it would not be too late for you to obtain a visa before your planned departure for China. Usually you should apply about one month in advance of your intended date of entry into China, and do not apply three months earlier.

Personal Information

The Center collects personal information about you ("Personal Information") in order to provide the Services. Before submitting an application to the Center, you should read our Privacy Policy available on our official website and at our Center. The Privacy Policy describes how your personal information is collected, used and disclosed and your rights in relation to accessing and correcting that information. You agree to the collection, use and disclose of your personal information in accordance with our Privacy Policy.

Loss/Damage/Theft of your Documents

To the maximum extent permitted by law, the Center shall not be liable to you or anyone claiming through you for any loss or damage suffered in relation to your application documents and supporting documents including but not limited to your passport (the Documents) being lost, stolen, damaged, misplaced or not returned to you.

Without prejudice to any other term in the Terms, and without any admission of liability whatsoever, if your passport is lost or damaged while it was in the Center's possession, the Center will reimburse you the standard fee charged by the applicable government department for a replacement passport upon the Center receiving from you a copy of proper receipt.

Disclaimer and Limitation of Liability

To the maximum extent permitted by law, the Center expressly disclaims all conditions and warranties, express or implied, in connection with the Services other than any conditions or warranties expressly stated in the Terms. Where the law precludes such exclusion and implies certain terms, conditions and/or warranties into the Terms ("Implied Terms"), the liability of the Center for a breach of such Implied Term shall be limited, at the option of the Center, to any one or more of the following:

(a) the re-supply of the relevant Services; or

(b) the cost of the re-supply of the relevant Services; or

(c) any amount paid by you to the applicable government department or authority in respect of the relevant Services (upon being provided with proper receipt).

You agree that in no circumstances will the Center be liable to you or anyone claiming though you for any indirect, incidental, special and/or consequential losses or damages of whatever nature, or for loss or profits, loss of opportunity, loss of business or goodwill or interruption of business arising out of or in connection with your application and/or any of the Services, however arising or where so ever situated, whether or not relating to any act, error, omission or negligence by the Center or any officer, agent, employee and representative of the Center.

Force Majeure

Strictly in line with relevant laws of Canada, the Center shall take every precaution to ensure the safety of the personal information and documents of the applicants, but it shall not be liable to your or anyone claiming through you for any consequences due to events or acts beyond its reasonable control such as natural disasters or an act of God, unforeseen accidents, robbery, and failure of equipment of the Center (Force Majeure), which may occur when the information and documents are in transit between the Center and the Consulate General.

Indemnity

You agree to indemnify and hold the Center, its management personnel, agents, employees and representatives harmless from any claim, loss, damage, proceeding or other action (including but not limited to reasonable legal fees), made by any third party arising out of a breach of the Terms by you, or a violation by you of any law of any country or of the rights of a third party.

Variation of Terms

Any of the Terms may be amended or varied by the Center at any time by notice to the public.

Governing Law

All Terms shall be governed by laws in the respective state where the center is located.

Instructions

Please note:

1. Items marked with a red asterisk “*”must be filled out, without which, you will not be able to proceed to the next step. 

2. All items must be carefully filled in and double check before submission. Once submitted, no item can be corrected.

3. After submission, please sign on the printed application form. The printed papers include 3 pages. Page 1 and 2 are the visa application form and the bar code is on page 3, which is for the Center’s further processing. All the 3 pages must be brought to the Center.
